Enrollment Process:

1. Obtain an enrollment packet from the school office or print forms below. This packet includes:

Information Sheet	Bus Route Agreement Form
Tuition & Fees Sheet	Parent Participation Agreement
Student Application 2008-2009	Student Narrative & Questionnaire (6^th-12^th)
OK Immunization Requirements	Student Statement of Cooperation
Parental Release & Agreement Form	Reference Forms (6^th – 12^th)
Authorization of Medical Care	Adult Friend
RenWeb Directory	Educator

NOTE: No student will be considered enrolled without completing the above enrollment process. Should it be determined by the administration that Victory Life Academy is not able to meet the educational needs of the student and is not able to enroll him/her, the enrollment fee(s) will be refunded in full. Otherwise, all enrollment fees are non-refundable.
